Use the email address registered with your NAHA case. A one-time six-digit code will be sent to verify your identity. The code expires after 10 minutes.

Documents requiring your signature
Review each document completely. Your typed legal name, verified email, date, time and document fingerprint become part of the electronic-signature evidence.
Upload requested documents
Files are stored in the private medical-document area for your selected case. Do not use email to send medical files.
Complete all required electronic signatures before uploading medical documents.
Maximum 5 files, 10 MB each and 25 MB combined.
